Tod,
I have had requests and have provided certificates of insurance, but it has
only happened 2-3 times in 25 years. I do not recall that I had to provide
the venue on the certificate.
If you carry insurance thru CDSS or Callerlab, they should be able to help
provide certificate.

This is the first such request I have received as caller hired by an
institution. Any suggestions on how to handle this request? Is this
becoming common? Is this an appropriate topic for this group? Thank you!
